A legal workflow may have a sound standard path while still receiving matters that do not fit it. If each exception is handled only within the individual file, the team has little view of whether the pattern is changing.
An exception review brings those records together. It can group deviations by issue, source, decision and outcome, then ask whether the standard path needs another branch, clearer intake information or a different approval boundary.
The aim is not to eliminate exceptions. Some should remain exceptional because they require judgment. The review makes that distinction visible and gives the legal team a record of what the operating model cannot safely decide on its own.
