An escalation note records why an AI-assisted workflow stopped, what evidence was missing and what a reviewer needs to decide.
An AI-assisted workflow should not treat every unresolved issue as a failure. Some matters are meant to stop because the available context is incomplete or the issue sits outside the approved path.
An escalation note makes that stop useful. It can identify the boundary that was crossed, the documents considered, the missing information and the decision requested from the reviewer. The note is not a legal conclusion. It is a structured handoff.
Over time, recurring notes may show that an intake question is unclear or that a playbook needs another branch. The record should not be used to expand automation automatically. A repeated exception may still require a human decision about risk tolerance.
